LMS-2254 Insert and edit Table notes in SI/SSIs

Summary of feature request

Enable users to insert notes into tables that will appear at the end of the table rather than at the foot of the page. It should be possible to insert multiple references within the one table to the one note. It should also be possible to insert notes that are not referenced by particular cells in the table.

Once they have been inserted, it should be possible to edit the notes.
